Does anyone know the address of the Head Office/Customer Services for British School of Motoring

I paid by credit card for a Practical Driving Test via BSM, but after waiting for over three weeks for the date I phoned customer service to cancel it. Whilst on the phone I gave them my credit card details and I did that back in late June 2011. I have just looked at this months (August) credit card statement and I have yet to receive a refund. How do I get the refund (£66.00) if I do not know the address of either Customer Service or the Head Office of BSM. I do not require the telephone number. Many thanks for your help.
